What We Offer

Intake Appointments

Intake appointments are your first step toward getting the support you need. During this initial meeting, our friendly Community Support Worker will ask about your background, current concerns, goals and any immediate needs to help match you with the right programs or services.

You can expect questions about health, family, housing, income and accessibility needs — all information is kept confidential and used to build a personalized care plan. You don’t need to have all the answers; it’s okay to bring notes or a support person. If you need interpretation, accommodations, or have questions beforehand, let us know and we’ll make the intake process as comfortable and helpful as possible.


Individual Support

One-on-one guidance tailored to each individual’s unique needs. Our Community Support Worker will work with clients to build strengths, address challenges, and connect with community resources that promote safety, stability and growth. Services may include goal-focused counselling, skill-building (communication, emotional regulation, problem solving), advocacy, and assistance navigating systems like education, health care and social services. We prioritize respectful, trauma-informed care that empowers young people and their families to reach their goals and stay connected to their community.


Employment Support

We help young people and their families access the tools and guidance needed to find and keep employment. Services may include job search assistance, résumé and cover letter development, interview preparation, skills and career assessments, connections to local employers and apprenticeships, and advocating for workplace accommodations. We offer one-on-one coaching, supported job placements, and follow-ups to ensure stability and success on the job. Services are client-centred, trauma-informed, and tailored to each person’s strengths, goals, and circumstances.


Independence / Transitional Support

Independence grows from confidence, skills, and a safety net. Our transitional support helps young people move from dependence to self-reliance by offering practical life-skills coaching, housing navigation, employment preparation, and emotional support tailored to each person’s needs. We partner with families and community resources to build stability while promoting decision-making, financial literacy, and healthy relationships. The result is a steadier journey into adulthood — safer, more prepared, and supported every step of the way.

We also support youth who are moving on to secondary school, are facing changes in their home life, or through any of life’s other transitional periods.


Follow-up Appointments

Follow-up appointments help us track progress, adjust care, and catch concerns early. They give you a chance to review how your care plan is working, ask questions, and plan next steps. Please come prepared with updates you think may be relevant to your care, and any questions or goals you want to discuss.
